Abstract

Objective: To study symptom clusters and functional performance inpatients with chronic obstruction pulmonary disease (COPD), and toexamine factors predicting the functional performance in COPD patientsincluding symptom cluster, severity, comorbidity, perceived health status,and social support. Methods: In this predictive correlational design. Thesample was 110 COPD patients from a multi-stage random sampling from4 hospitals in the eastern region of Thailand. The research instrumentswere 7 questionnaires and recording forms: personal illness data, comorbidity,physical and psychological symptom, health status perception,social support, and functional performance. Data were analyzed by usingfactor analysis, Pearson’s product moment correlation and multipleregression analysis. Results: Symptom clusters in persons with COPDwere classified into the two groups. Symptom cluster I (physical symptomcluster) including dyspnea, fatigue and insomnia, and symptom cluster II(psychological symptom cluster) including anxiety, and depression, stage ofdisease severity, perceived health status, and social support, weresignificantly correlated with functional performance in persons with COPD(P < 0.05). Moreover, perceived health status, and stage of diseaseseverity could predict functional performance in persons with COPD andexplain 58.6 % of the variance (P < 0.001). Conclusions: Perceived healthstatus and disease severity could predict functional performance in patientswith COPD. Developing and testing effective stage of disease severity andpromoting health perception management program could potentiallyimprove functional performance and quality of life of this group of patients.Keywords: chronic obstructive pulmonary disease, COPD, health statusperception, social support, functional performance
