ความสัมพันธ์ระหว่างวิธีการสอบเข้าศึกษาและคะแนนรายวิชาการสอบเข้ากับผลสัมเร็จทางการเรียนของนิสิตเภสัชศาสตร์ มหาวิทยาลัยศรีนครินทรวิโรฒ Relationship between Admission Criteria and Score of Each Subject and Academic Success among Pharmacy Students of Srinakharinwirot University

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างวิธีการสอบเข้าศึกษาและคะแนนการสอบเข้ากับผลสำเร็จทางการเรียนของนิสิตเภสัชศาสตร์ มหาวิทยาลัยศรีนครินทรวิโรฒ โดยใช้วิธีการศึกษาข้อมูลย้อนหลังจากระบบทะเบียนนิสิตและงานทะเบียนการศึกษา ของกลุ่มตัวอย่างนิสิตที่เข้าศึกษาในช่วงปีการศึกษา 2562-2565 จำนวนทั้งสิ้น 299 คน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา การทดสอบไคสแควร์ (Chi-square test) การทดสอบความแตกต่างแบบ Kruskal–Wallis และการวิเคราะห์สหสัมพันธ์ของเพียร์สัน (Pearson correlation) ผลการศึกษา ไม่พบความแตกต่างที่มีนัยสำคัญทางสถิติของคะแนนเฉลี่ยสะสม (GPA) และอัตราการออกกลางคันระหว่างกลุ่มนิสิตที่มีวิธีเข้าศึกษาต่างกัน คือ วิธีโควตา วิธี กสพท (กลุ่มสถาบันแพทยศาสตร์แห่งประเทศไทย) และวิธีการรับตรงร่วมกัน (Admission) (p > .05) แม้กลุ่มรับตรงมีค่า GPA เฉลี่ยสูงกว่ากลุ่มอื่นเล็กน้อย และกลุ่มโควตามีสัดส่วนการออกกลางคันสูงกว่าในเชิงร้อยละ สำหรับความสัมพันธ์ระหว่างคะแนนสอบเข้ากับผลสำเร็จทางการเรียน พบว่าเฉพาะคะแนนสอบเข้าวิชาคณิตศาสตร์1 และฟิสิกส์ มีความสัมพันธ์เชิงบวกกับ GPA อย่างมีนัยสำคัญทางสถิติ (r = -0.177 และ -0.55 ตามลำดับ, p < .05) ขณะที่คะแนนวิชาเคมี ชีววิทยา และภาษาอังกฤษ ไม่พบความสัมพันธ์ที่มีนัยสำคัญ จึงสรุปผลการศึกษาได้ว่า ช่องทางการคัดเลือกเข้าศึกษาไม่ได้เป็นตัวบ่งชี้ผลสัมฤทธิ์ทางการเรียนที่มีนัยสำคัญและความสามารถด้านคณิตศาสตร์มีบทบาทต่อผลสัมฤทธิ์ของการศึกษาในหลักสูตรเภสัชศาสตรบัณฑิต

บทคัดย่อ

ทักษะการสื่อสารและการให้คำปรึกษาด้านยา เป็นทักษะที่สำคัญสำหรับช่วยค้นหาและแก้ไขปัญหาด้านการใช้ยาของผู้ป่วยที่ยังคงมีอัตราสูงในปัจจุบัน โดยเฉพาะในผู้สูงอายุที่มีโรคเรื้อรังและการใช้ยาหลายขนานร่วมกัน ซึ่งนิสิตเภสัชศาสตร์บางรายยังขาดทักษะดังกล่าวอยู่ วัตถุประสงค์: เพื่อประเมินทักษะของนิสิตเภสัชศาสตร์ชั้นปีที่ 5 มหาวิทยาลัยศรีนครินทรวิโรฒ ในการให้คำปรึกษาด้านยาแก่ผู้ป่วยจากสถานการณ์จำลองทางเภสัชกรรมปฏิบัติโดยมุ่งเน้นผู้ป่วยเป็นศูนย์กลาง วิธีการศึกษา: การวิจัยแบบสำรวจเชิงพรรณนาในนิสิตเภสัชศาสตร์ชั้นปีที่ 5 มหาวิทยาลัยศรีนครินทรวิโรฒ ปีการศึกษา 2563 ทั้งหมดจำนวน 67 คน โดยให้นิสิตสวมบทบาทเป็นเภสัชกรและให้คำปรึกษาด้านยาแก่ผู้ป่วยตามสถานการณ์จำลองทางเภสัชกรรมปฏิบัติที่กำหนด แสดงผลการศึกษาเป็นความถี่และร้อยละ ผลการศึกษา: นิสิตเภสัชศาสตร์ชั้นปีที่ 5 มหาวิทยาลัยศรีนครินทรวิโรฒ ปีการศึกษา 2563 มีผลสัดส่วนประเด็นคำพูดที่เหมาะสมเฉลี่ยคิดเป็นร้อยละ 44.07±8.67 และสัดส่วนประเด็นคำพูดที่ไม่เหมาะสมเฉลี่ยคิดเป็นร้อยละ 3.23±6.64 ร้อยละประเด็นคำพูดที่เหมาะสมของนิสิตหลักสูตรวิทยาศาสตร์เภสัชกรรมเท่ากับ 42.21±7.08 และนิสิตหลักสูตรการบริบาลทางเภสัชกรรมคิดเป็นร้อยละ 47.62±10.35 ร้อยละประเด็นคำพูดที่ไม่เหมาะสมของนิสิตหลักสูตรวิทยาศาสตร์เภสัชกรรม เท่ากับ 3.79±7.07 และนิสิตหลักสูตรการบริบาลทางเภสัชกรรมคิดเป็นร้อยละ 2.17±5.74 สรุป: นิสิตยังคงมีปัญหาในการให้คำปรึกษาด้านยาแก่ผู้ป่วยโดยยึดผู้ป่วยเป็นศูนย์กลาง ทั้งในแง่การค้นหาปัญหา การเชื่อมโยงความรู้พื้นฐานด้านยาในการอธิบายผู้ป่วย และการแก้ไขปัญหาโดยมุ่งเน้นผู้ป่วยเป็นศูนย์กลาง ดังนั้นจึงควรนำผลการศึกษาไปพัฒนาและปรับปรุงหลักสูตรการศึกษา เพื่อพัฒนาทักษะและความรู้ในการให้คำปรึกษาด้านยาโดยมุ่งเน้นผู้ป่วยเป็นศูนย์กลางของนิสิตต่อไป

Abstract

Communication and medication counseling skills are important for identifying and solving drug-related problems, which remain highly prevalent, especially among elderly patients with chronic diseases and polypharmacy. However, some pharmacy students still lacked these skills. Objective: To evaluate medication counseling skill of fifth-year pharmacy students, Srinakharinwirot University, Academic Year 2020, in a pharmaceutical simulation practice with a patient-centered approach. Method: This observational descriptive study was conducted among 67 fifth-year pharmacy students enrolled at the Faculty of Pharmacy, Srinakharinwirot University during the 2020 academic year. The students were assigned the role of pharmacists giving drug counseling to patients. The results were presented as frequencies and percentages. Result: the fifth-year pharmacy students, Srinakharinwirot University had an average ratio of appropriate speech issues 44.07±8.67% and the average proportion of inappropriate speech issues was 3.23±6.64%. The inappropriate speech of pharmaceutical science students was 42.21±7.08%, and pharmaceutical care students was 47.62±10.35%. The proportion of inappropriate speech of pharmaceutical science students was 3.79±7.07% and pharmaceutical care students were 2.17±5.74%. Conclusion: The fifth-year pharmacy students of Srinakharinwirot University still had problems in giving drug counseling to patients based on the patient-centered approach, either finding problems or linking basic knowledge of medicine in explaining patients with problem solving. These finding should be used to develop the curriculum to improve students’ skills and knowledge in patient-centered medication counseling.

ผลของโปรแกรมกระตุ้นศักยภาพสมองด้านการรู้คิดต่อคลื่นไฟฟ้าสมองในผู้สูงอายุที่มีภาวะสมองเสื่อม: การศึกษาเบื้องต้นEffects of the Cognitive Stimulation Therapy on Electroencephalogram among the Older Adults with Dementia: A Preliminary Study

Objective: To determine effects of a Cognitive Stimulation Therapy (CST) program on the Electroencephalogram (EEG) in demented older adults.

Methods: This quasi-experimental study use the one-group pretest-posttest design. The participants were 30 older adults who had Mini-Mental State Examination (MMSE) score of mild to moderate dementia in the government elderly nursing home selected by simple random sampling. The study was conducted from February to April 2024. The participants received the 14 theme sessions of CST program based on the Isan cultural context twice a week over 7 weeks. A 5-minute resting-state EEG (eyes open) was collected by the Emotiv EPOCX Neuroheadset for pre- and post-test measurements. The EEG relative powers for theta and alpha oscillations were performed offline in MATLAB (version R2023b) with EEGLAB plugin Darbeliai (version 2023.0). A paired t-test one group pre-test and post-test design was used for analyses of theta and alpha relative powers.

Results: The intervention yielded improvement, but the effect size was small (d = 0.38). Significant pre- to post-intervention increases were observed in relative EEG power following completion of the CST program. The average of each frequency band was calculated across the average of fronto-central (FC5-FC6) electrodes. The theta (4-8 Hz) wave, as a low-frequency wave, was significantly decreased (p <.05), and also the Alpha wave (8-12 Hz), as a high-frequency wave pattern, significantly increased (p < .01) at the fronto-parietal lobe of both cerebral hemispheres.

Conclusion: The findings suggest that the CST program, adapted to the Isan cultural context, is an effective group intervention to improve cognitive decline in demented older adults. Nurses should be trained to have adequate competency to provide the CST program. Further research is required to promote the maintenance longer-term effects of cognitive intervention.

การซักประวัติไม่ครบถ้วนและการประเมินโรคที่ผิดพลาด อาจนำไปสู่การเลือกจ่ายยาที่ไม่เหมาะสมกับผู้ป่วย ซึ่งเป็นปัญหาที่พบได้บ่อยในระหว่างการฝึกปฏิบัติงานของนิสิตเภสัชศาสตร์ วัตถุประสงค์: เพื่อประเมินความสามารถของนิสิตเภสัชศาสตร์ชั้นปีที่ 4 มหาวิทยาลัยศรีนครินทรวิโรฒ (มศว) ในการประยุกต์ใช้ความรู้และทักษะในการซักประวัติผู้ป่วยจากสถานการณ์จำลองทางเภสัชกรรมชุมชน วิธีการศึกษา: การวิจัยแบบสำรวจเชิงพรรณนาในนิสิตเภสัชศาสตร์ชั้นปีที่ 4 มศว ปีการศึกษา 2563 ทั้งหมดจำนวน 82 คน โดยให้นิสิตสวมบทบาทเป็นเภสัชกรประจำร้านยา ทำการซักประวัติผู้ป่วยและสรุปอาการเจ็บป่วยตามสถานการณ์จำลองทางเภสัชกรรมชุมชนที่กำหนด ซึ่งมีผู้วิจัยเป็นผู้ให้คะแนนตามเกณฑ์ที่ตั้งไว้ แสดงผลการศึกษาเป็นความถี่ ร้อยละ และมาตราวัดแบบการประมาณค่า 5 ระดับ ตามวิธีของลิเคริท ผลการศึกษา: นิสิตเภสัชศาสตร์ชั้นปีที่ 4 มศว ปีการศึกษา 2563 ส่วนมากจัดอยู่ในกลุ่มที่มีทักษะปานกลาง คิดเป็นร้อยละ 59.72 มีค่าเฉลี่ยของคะแนนเท่ากับ 21.12 ± 4.28 คะแนน นิสิตในหลักสูตรการบริบาลทางเภสัชกรรมจำนวน 28 คน มีคะแนนการซักประวัติฯผู้ป่วยเฉลี่ยมากกว่านิสิตในหลักสูตรวิทยาศาสตร์เภสัชกรรมซึ่งมีจำนวน 54 คน โดยคิดเป็น 21.61 ± 4.22 คะแนน และ 21.06 ± 4.34 คะแนนตามลำดับ โดยคำถามที่ผู้เข้าร่วมวิจัยถามมากที่สุด คือ ข้อ 3 เป็นอะไรมา/มีอาการอย่างไรมา และข้อ 4 มีไข้/ได้ทำการวัดอุณหภูมิตัวเองไหม/ขอวัดอุณหภูมิร่างกาย คิดเป็นร้อยละ 100 (82/82 คน) คำถามที่ไม่มีผู้เข้าร่วมวิจัยคนใดถาม คือ ข้อ 6 นอกจากแน่นจมูกแล้วมีรู้สึกกดเจ็บ บริเวณหน้า รอบดวงตา ปวดฟัน หรือรู้สึกมีลมหายใจเหม็นๆ ผิดปกติบ้างไหม (0/82 คน) ซึ่งเป็นคำถามที่ใช้ความรู้ในด้านการประเมินแยกโรค จากการประเมินความสามารถในการประยุกต์ใช้ความรู้และทักษะในการซักประวัติผู้ป่วย สรุป: นิสิตส่วนใหญ่มีทักษะการซักประวัติผู้ป่วยและความสามารถในการประยุกต์ใช้ความรู้ระดับปานกลาง แต่ไม่สามารถนำความรู้ทั้งหมดมาประยุกต์ใช้ในการซักประวัติผู้ป่วยได้ โดยนิสิตยังไม่สามารถคิดและถามคำถามที่เฉพาะเจาะจงต่อโรคที่สัมพันธ์กับข้อมูลและอาการของผู้ป่วยที่ได้จากการซักประวัติ อีกทั้งยังไม่สามารถคิดย้อนกลับเพื่อหาสาเหตุและความเป็นไปได้ของโรคหรืออาการของผู้ป่วยว่าสามารถเกิดจากสาเหตุใดได้บ้าง กล่าวคือนิสิตยังไม่สามารถดึงความรู้มาใช้ได้อย่างครอบคลุม ดังนั้นจึงควรนำผลการศึกษาไปพัฒนาและปรับปรุงหลักสูตรการศึกษาเพื่อพัฒนากระบวนการคิดและทักษะในการซักประวัติผู้ป่วยของนิสิตต่อไป

Abstract

Incomplete patient history taking and inaccurate disease assessment may lead to inappropriate medication selection, a problem commonly observed during pharmacy students' clinical training. Objective: To investigate the abilities of pharmacy students at the Faculty of Pharmacy, Srinakharinwirot University, to integrate knowledge and skills in history taking in scenario of community pharmacy. Method: This observational descriptive study was conducted among 82 fourth-year pharmacy students enrolled at the Faculty of Pharmacy, Srinakharinwirot University during the 2020 academic year. The students were assigned the role of the community pharmacy to take patient histories and summarize the patients’ symptoms. The researcher evaluated their performance using predetermined criteria. The results were presented as frequencies, percentages and five-point rating Likert scale. Result: The majority of the students were considered to be moderate skills in history taking and knowledge integration. The overall average score was 21.12 ± 4.28 (59.72%). Moreover, 28 people of pharmaceutical care students had a history taking score more than those of 54 people pharmaceutical science students, the scores were 21.61 ± 4.22, and 21.06 ± 4.34, respectively. The questions asked by the participants the most were: item 3, what is wrong/what is your symptom?, and item 4, Have you taken your temperature? / Ask for body temperature, which are considered to be 100% (82 of 82). The question that the participants did not ask was: item 6, apart from nasal congestion, there is a feeling of pain around your eyes, face, teeth, or unpleasant-smelling; 0 of 82, which used symptoms evaluation knowledge. Conclusion: The most of the students were considered to be moderate level of patient history taking skills, but they cannot gather the information from patients to analyze the patient’s problem properly especially in symptoms evaluation knowledge. Additionally, they not only cannot predict the possibility of the symptoms but also do not understand the patient’s conditions. These finding should be used to develop the curriculum to improve students’ thinking process and history taking skills.

การศึกษาวิเคราะห์พิกัดยาสมุนไพรในคัมภีร์สรรพลักษณะ สรรพคุณ แลมหาพิกัด เปรียบเทียบกับพิกัดยาในบัญชียาหลักแห่งชาติ พิกัดยาในคลินิกอายุรเวทแพทย์แผนไทยประยุกต์ ศิริราช และหลักฐานเชิงประจักษ์

บทคัดย่อ

วัตถุประสงค์ : เพื่อศึกษาวิเคราะห์พิกัดยาสมุนไพรในคัมภีร์สรรพลักษณะ สรรพคุณ แลมหาพิกัด เปรียบเทียบกับพิกัดยาในบัญชียาหลักแห่งชาติ พิกัดยาในคลินิกอายุรเวทแพทย์แผนไทยประยุกต์ ศิริราช และสืบค้นหลักฐานเชิงประจักษ์

วิธีการ : เป็นการวิจัยเชิงคุณภาพ (Qualitative Research) จากเอกสารและแหล่งข้อมูลปฐมภูมิ (Primary Sources) 3 แหล่ง คือ คัมภีร์สรรพคุณลักษณะฯ พิกัดยาในประกาศคณะกรรมการพัฒนาระบบยาแห่งชาติ นำข้อมูลที่ได้มาเปรียบเทียบ และสืบค้นฐานข้อมูลพิกัดยาที่มีการใช้ในคลินิกอายุรเวทแพทย์แผนไทยประยุกต์ ศิริราช และหลักฐานเชิงประจักษ์ของพิกัดยาสมุนไพรที่น่าสนใจในฐานข้อมูล PubMed

ผลการศึกษา : พบพิกัดยาในคัมภีร์สรรพคุณลักษณะฯ 96 พิกัด จำแนกหมวดหมู่พิกัดยาได้ 10 กลุ่ม กลุ่มพิกัดที่พบมากที่สุดคือ พิกัดยา 3 สิ่ง รองลงมาคือ จุลพิกัด และ พิกัดยา 5 สิ่ง ตามลำดับ เมื่อวิเคราะห์แต่ละพิกัดพบว่าพิกัดยาที่พบมาก 5 ลำดับ คือ (1) ตรีกฏุก (2) ตรีผลา (3) จันทน์ทั้ง 2 (4) โกฐทั้ง 5 และ (5) เบญจกูล และเมื่อวิเคราะห์พิกัดยาที่พบในตำรับยาในบัญชียาหลักแห่งชาติ พบตำรับยาที่มีพิกัดยาเป็นส่วนประกอบ 34 ตำรับจาก 74 ตำรับ ประกอบด้วยพิกัดยา 28 พิกัด เมื่อเปรียบเทียบพิกัดยาจากทั้ง 2 แหล่ง พบพิกัดยาตรงกัน 19 พิกัด และเมื่อนำข้อมูลพิกัดยาไปสืบคนต่อในฐานข้มูลการใช้ยาของคลินิกอายุรเวทแพทย์แผนไทยประยุกต์ ศิริราช พบว่ามี 4 พิกัดที่ใช้มากที่สุดคือพิกัดตรีผลา รองลงมาพิกัดเกสรทั้ง 5 เบญจกูล และห้าราก จากนั้นได้ทำการสืบค้นข้อมูลเวชศาสตร์เชิงประจักษ์เกี่ยวกับพิกัดยาสมุนไพรที่น่าสนใจ พบหลักฐานเชิงประจักษ์ที่เกี่ยวข้องกับสรรพคุณพิกัดยา 3 พิกัด

สรุป : ผลการวิเคราะห์และเปรียบเทียบกับพิกัดยาสมุนไพรในแหล่งข้อมูลปฐมภูมิ และสืบค้นพิกัดยาที่ใช้ในคลินิกอายุรเวทแพทย์แผนไทยประยุกต์ ศิริราช พบมีการใช้พิกัดยาอยู่จริงในทางคลินิกปัจจุบัน เมื่อนำพิกัดไปสืบค้นหลักฐานเชิงประจักษ์ ซึ่งหลักฐานเชิงประจักษ์ยังมีน้อย แต่มีประโยชน์ต่อวิชาชีพการแพทย์แผนไทย เป็นหลักฐานเชิงประจักษ์ใหม่ที่ใช้อ้างอิงในการเรียนการสอน และการทำเวชปฏิบัติด้านการแพทย์แผนไทยได้

โรคจิตเภท (schizophenia) เป็นโรคทางจิตเวชที่เซับซ้อนและรุนแรงส่งผลต่อคุณภาพชีวิตของผู้ป่วย อาการของโรคจิตเภทประกอบด้วยอาการด้านบวก (positive symptoms) เช่น อาการประสาทหลอน (hallucination) อาการหลงผิด (delusion) เป็นต้น อาการด้านลบ (negative symptoms) เช่น พูดน้อย แยกตัวจากสังคม เป็นต้น อาการด้านการรู้คิด (cognitive symptoms) เช่น ความจำลดลง ความสามารถในการวางแผนและแก้ปัญหาบกพร่อง เป็นต้น ประชากรทั่วโลกมีความชุกในการเป็นโรคจิตเภทเท่ากับร้อยละ 0.33-0.75 โดยโรคจิตเภทเป็นสาเหตุของการเกิดภาวะทุพลภาพ 1 ใน 15 ลำดับแรกของทั่วโลกและยังเพิ่มความเสี่ยงในการเสียชีวิตตั้งแต่ยังอายุน้อยเมื่อเทียบกับประชากรทั่วไป ยา cobenfy® ประกอบด้วยตัวยาสำคัญ 2 ตัว คือ xanomeline และ trospium chloride โดย xanomeline มีกลไกการออกฤทธิ์โดยไปกระตุ้นตัวรับมัสคารินิกชนิด M1 และ M4 (M1/M4 muscarinic receptor) ที่ระบบประสาทส่วนกลางส่งผลให้สามารถควบคุมอารมณ์และพฤติกรรมได้รวมถึงเพิ่มประสิทธิภาพด้านการเรียนรู้และความจำ ส่วนยา trospium chloride ทำหน้าที่เป็นสารต้านตัวรับมัสคารินิกชนิด M2 และ M3 (M2/M3 muscarinic antagonist) เฉพาะที่ระบบประสาทส่วนปลาย จึงช่วยทำให้ลดการเกิดอาการไม่พึงประสงค์ทางด้านหัวใจและหลอดเลือดและทางเดินอาหารที่อาจเกิดจากการใช้ยา xanomeline ได้โดยไม่ส่งผลต่อการออกฤทธิ์ของ xanomeline ในระบบประสาทส่วนกลางเนื่องจาก trospium chloride ไม่สามารถผ่านแนวกั้นระหว่างเลือดและสมอง (Blood-brain barrier) ได้ จากการศึกษาทางคลินิกและการวิเคราะห์อภิมาน (meta-analysis) ที่ทำการศึกษาถึงประสิทธิภาพและความปลอดภัยของยา xanomeline-trospium chloride พบว่ามีประสิทธิภาพในการลดอาการของโรคจิตเภททั้งอาการทางด้านบวกและอาการทางด้านลบได้อย่างมีนัยสำคัญทางสถิติเมื่อเทียบกับยาหลอกนอกจากนี้ยังไม่ส่งทำให้เกิดอาการเคลื่อนไหวผิดปกติจากการใช้ยารวมถึงไม่ทำให้น้ำหนักตัวเพิ่มขึ้นอีกด้วย

Schizophrenia is a complex psychiatric disorder characterized by positive symptoms (such as hallucinations, delusions), negative symptoms (such as alogia, social withdrawal), and cognitive impairment (such as memory loss). It is a leading cause of disability worldwide and significantly reduces life expectancy. Cobenfy® is an alternative therapy consisting of Xanomeline and Trospium chloride. Xanomeline acts as a selective M1 and M4 muscarinic receptor agonist, effectively suppressing excessive dopamine release to stabilize mood and behavior. Trospium chloride acts as a M2 and M3 muscarinic antagonist in peripheral tissues, which reduces the adverse effects caused by Xanomeline. Since Trospium cannot cross the blood-brain barrier, it does not interfere with Xanomeline’s therapeutic action in the central nervous system. Clinical studies and meta-analyses demonstrate that Xanomeline and Trospium chloride significantly improves both positive and negative symptoms compared to a placebo. Notably, this medication does not cause common side effects associated with traditional antipsychotics, such as movement disorders or weight gain.

In this first issue of volume 21, the Journal has been proudly presenting five studies. The disciplines and issues of these research papers were somewhat diverse from applied laboratory work, to clinical practice both for pharmacy and nursing.

In this challenging endeavor of the Thai Pharmaceutical and Health Science Journal, we are hopeful to better the quality of the articles published. We urge more submissions from international research community, regional and global. We would like to thank in advance for any prospective submissions.
