Abstract

Objectives: To study effects of medication therapy management (MTM) onglycemic control in type 2 diabetic outpatients by pharmacist. Method:Eighty-six type 2 diabetic patients at Nongbua hospital, Nakornsawan, wereenrolled in the study during August 2010 to September 2011. Patients wererandomized to receive either medication therapy management (MTM)(study group) or regular service (control group), with 43 patients each.MTM was carried out by the pharmacist researcher who identified, resolvedand prevented medication-related problems, educated patients aboutdisease and life-style modification, and followed the refill guideline. Bothgroups were evaluated for HbA1C at month 0 and 3 (end of study), andFPG at moth 0, 1, 2 and 3. Results: At the end of study, HbA1C in studygroup (8.1 ± 1.2%) was significantly lower than that in control group (8.8 ±1.3%) (P < 0.05). Difference in FPG in study and control groups was notstatistically significant. However fasting plasma glucose levels at month 1, 2and 3 in study group were significantly decreased (P < 0.05). In studygroup, of 172 MTM sessions carried out (from 172 visits), 114 drug relatedproblems (DRPs) were identified by pharmacist. Of these DRPs, 104 wereresolved by pharmacist and another 10 were referred to physician. Mostaccountable DRPS were non-compliance. Conclusion: Medication therapymanagement for type 2 diabetic outpatients by the pharmacist improvedpatients’ glycemic control and solved drug-related problems.Key words: medication therapy management, drug-related problem,diabetes, pharmacist
