Abstract

วัตถุประสงค์ : เพื่อประเมินผลการนำกระบวนการตรวจคัดกรองยีน Human leukocyte Antigen (HLA) ก่อนเริ่มยา allopurinol และ carbamazepine ไปใช้ในโรงพยาบาลแม่สอด ตลอดจนเปรียบเทียบสัดส่วนการตรวจพบ HLA-B*58:01 และ  HLA-B*15:02 ระหว่างผู้ป่วยชาวไทยและผู้ป่วยประชากรข้ามชาติ วิธีการศึกษา: การวิจัยเชิงปฏิบัติการ ที่ขับเคลื่อนการพัฒนากระบวนการตรวจคัดกรองยีน Human leukocyte Antigen (HLA) ในผู้ป่วยที่มีคำสั่งเริ่มยา allopurinol และ carbamazepine ผ่านคณะกรรมการเภสัชกรรมและการบำบัดของโรงพยาบาลแม่สอด ดำเนินการระหว่างวันที่ 1 มกราคม 2568 – 31 ตุลาคม 2568 เครื่องมือวิจัยประกอบด้วยแนวทางการคัดกรองยีนกลุ่ม HLA และแบบบันทึกข้อมูลผู้ป่วย วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและเปรียบเทียบสัดส่วนการตรวจพบ HLA-B*58:01 และ HLA-B*15:02  ระหว่างผู้ป่วยชาวไทยและผู้ป่วยประชากรข้ามชาติด้วย Chi-square test หรือ Fisher's exact test ผลการศึกษา: ผู้ป่วยที่มีคำสั่งเริ่มยา allopurinol 424 ราย carbamazepine 19 ราย ได้รับการตรวจคัดกรองยีน Human leukocyte Antigen (HLA) ก่อนเริ่มยาร้อยละ 78.07 และ 36.84 ตามลำดับ พบอาการไม่พึงประสงค์ทางผิวหนังชนิดไม่รุนแรงจากยา allopurinol    6 รายและยา carbamazepine 1 ราย เมื่อเปรียบเทียบสัดส่วนการตรวจพบ HLA-B*58:01 พบว่าผู้ป่วยชาวไทยมีสัดส่วนการตรวจพบ HLA-B*58:01 ร้อยละ 13.50 และผู้ป่วยประชากรข้ามชาติมีสัดส่วนการตรวจพบ HLA-B*58:01 ร้อยละ 5.26 ซึ่งผู้ป่วยชาวไทยมีสัดส่วนสูงกว่าอย่างมีนัยสำคัญทางสถิติ (p-value < 0.05) สัดส่วนการตรวจพบ HLA-B*15:02 ไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติ สรุป: การพัฒนากระบวนการตรวจคัดกรองยีน Human leukocyte Antigen (HLA) ก่อนเริ่มยา allopurinol และ carbamazepine ทำให้ผู้ป่วยได้รับการตรวจคัดกรอง HLA-B*58:01 และ HLA-B*15:02 ก่อนเริ่มยาร้อยละ 78.07 และ 36.84 ตามลำดับ โดยไม่พบอาการไม่พึงประสงค์ทางผิวหนังชนิดรุนแรง ได้แก่ SJS/DRESS/TEN ในช่วงที่ติดตาม และพบว่าผู้ป่วยชาวไทยมีสัดส่วนการตรวจพบ HLA-B*58:01 สูงกว่าผู้ป่วยประชากรข้ามชาติอย่างมีนัยสำคัญ ข้อค้นพบความแตกต่างของสัดส่วนการตรวจพบนี้และช่องว่างด้านสิทธิการรักษาของประชากรข้ามชาติ จะเป็นข้อมูลที่ใช้ในการพัฒนากระบวนการปฏิบัติในส่วนของนโยบายสาธารณสุขชายแดนต่อไป คำสำคัญ: allopurinol; carbamazepine; การตรวจคัดกรองทางเภสัชพันธุศาสตร์; อาการไม่พึงประสงค์ทางผิวหนังชนิดรุนแรง; ประชากรข้ามชาติ       Abstract Objectives: To evaluate the implementation of Human leukocyte Antigen (HLA) gene screening process prior to allopurinol and carbamazepine initiation at Mae Sot hospital. as well as to examine the differences in the detection of HLA-B*58:01 and  HLA-B*15:02 between Thai and migrant patients. Method: A action research driving the development of Human Leukocyte Antigen (HLA) gene screening prior to allopurinol and carbamazepine initiation through the Pharmacy and Therapeutics Committee of Mae Sot hospital. Conducted from January 1, 2025 to October 31, 2025. The research instruments consisted of the HLA gene screening guidelines and patient record forms. Data were analyzed using descriptive statistics, and the proportions of HLA-B*58:01 and HLA-B*15:02 detection between Thai and migrant patients were compared using the Chi-square test or Fisher's exact test. Results: Among the 424 patients prescribed allopurinol and 19 patients prescribed carbamazepine, 78.07% and 36.84% respectively, underwent Human Leukocyte Antigen (HLA) gene screening prior to drug initiation. Mild skin adverse reactions were observed with allopurinol in 6 cases and with carbamazepine in 1 case. When HLA-B*58:01 detection ratio was compared, Thai patients demonstrated a significantly higher detection ratio than migrant patients (p-value < 0.05). However, no significant difference was found in the HLA-B*15:02 detection ratio. Conclusion: The development of Human Leukocyte Antigen (HLA) gene screening process enabled 78.07% of patients to undergo HLA-B*58:01 screening and 36.84% to undergo HLA-B*15:02 screening prior to initiating allopurinol and carbamazepine, respectively. No severe cutaneous adverse reactions (SCARs) including SJS, DRESS, or TEN, were observed during the follow-up period. Thai patients demonstrated a significantly higher HLA-B*58:01 detection ratio than migrant patients. These findings regarding differences in detection proportions and the healthcare coverage gaps among migrant patients will serve as crucial data to further develop practice guidelines for border public health policies.
