ABC-FSN Analysis for Drug Inventory Management in a Teaching Hospital Pharmacy
การวิเคราะห์ ABC-FSN สำหรับการบริหารสินค้าคงคลังยาในโรงพยาบาลโรงเรียนแพทย์
Abstract
บทคัดย่อ วัตถุประสงค์: เพื่อประยุกต์ใช้การวิเคราะห์ ABC-FSN ในการจัดกลุ่มรายการยาและระบุแหล่งของความสูญเสียในคลังยา ได้แก่ ยาที่ไม่มีการหมุนเวียน (Dead stock) และการสำรองยาที่เกินความจำเป็น ซึ่งระบบ VEN เดิมไม่สามารถตรวจพบได้ พร้อมทั้งนำเสนอแนวทางการบริหารจัดการที่เหมาะสมสำหรับยาแต่ละกลุ่ม เพื่อลดความเสี่ยงและการสูญเสียในคลังยาของโรงพยาบาลโรงเรียนแพทย์ ทั้งนี้ แม้โรงพยาบาลจะได้นำการวิเคราะห์ VEN มาใช้บริหารคลังยาตั้งแต่ปี พ.ศ. 2563 แต่ในปี พ.ศ. 2568 ยังคงเผชิญกับภาระมูลค่าการแลกเปลี่ยนยาใกล้หมดอายุที่สูงถึง 10,644,348.40 บาท วิธีการศึกษา: การวิจัยเชิงพรรณนาแบบภาคตัดขวางโดยใช้ข้อมูลย้อนหลังจากระบบสารสนเทศของฝ่ายเภสัชกรรม ครอบคลุมรายการยาทั้งหมด 2,196 รายการ มูลค่าการบริโภคยารวมทั้งปี (Annual Drug Expenditure: ADE) 5,735,175,285.67 บาท ในช่วงเดือนมกราคมถึงธันวาคม พ.ศ. 2568 การวิเคราะห์ ABC จำแนกตามมูลค่าการบริโภคยาสะสมต่อปีโดยอิงหลักการ Pareto และ FSN จำแนกยาตามจำนวนเดือนที่มีการเบิกจ่ายจริงใน 12 เดือน แล้วสร้าง ABC-FSN matrix โดยเชื่อมข้อมูลทั้งสองชุดด้วยรหัสยา ผลการศึกษา: การวิเคราะห์ FSN พบยาหมวด F (Fast-moving) จำนวน 1,797 รายการ (ร้อยละ 81.8) หมวด S (Slow-moving) 285 รายการ (ร้อยละ 13.0) และหมวด N (Non-moving) 114 รายการ (ร้อยละ 5.2) การวิเคราะห์ ABC พบยาหมวด A จำนวน 273 รายการ (ร้อยละ 12.4) ใช้งบประมาณร้อยละ 69.95 ของ ADE รวม 4,011,990,820 บาท ABC-FSN matrix พบว่ากลุ่ม A-F (270 รายการ มูลค่า 3,990,223,083 บาท) ต้องการการควบคุมคลังสูงสุด กลุ่ม A-S (3 รายการ มูลค่า 21,767,738 บาท) เป็นยาชีววัตถุราคาสูงที่ต้องการนโยบายสั่งซื้อเฉพาะ ส่วน dead stock ทั้งหมด 114 รายการ (มูลค่า 10,155,936 บาท) กระจายอยู่ในกลุ่ม C-N ทั้งหมด โดยไม่พบ dead stock ในกลุ่ม A-N และ B-N สรุป: การประยุกต์ใช้เทคนิควิเคราะห์ ABC-FSN สามารถระบุรายการยาที่ไม่เคลื่อนไหว (Dead stock) ในกลุ่ม C-N จำนวน 114 รายการ จากทั้งหมด 2,196 รายการ (คิดเป็นร้อยละ 5.19) รวมมูลค่า 10,155,936 บาท และสามารถระบุยาชีววัตถุราคาสูงในกลุ่ม A-S จำนวน 3 รายการที่ควรใช้แนวทางการสั่งซื้อแบบทันเวลาพอดี (Just-in-time) ซึ่งรายการเหล่านี้ถือเป็นข้อจำกัดที่การวิเคราะห์ด้วย VEN เพียงระบบเดียวไม่อาจตรวจจับได้ ดังนั้น การบูรณาการเครื่องมือ ABC-FSN เข้ากับการวิเคราะห์ VEN จึงเป็นแนวทางที่จะช่วยยกระดับ การบริหารคลังยาของโรงพยาบาลโรงเรียนแพทย์ให้มีความแม่นยำและมีประสิทธิภาพสูงสุด คำสำคัญ: การวิเคราะห์ ABC; การวิเคราะห์ FSN; การบริหารยาคงคลัง; โรงพยาบาลโรงเรียนแพทย์; Dead stock ABSTRACT Objective: To apply ABC-FSN analysis to classify drug items and identify the sources of loss in the pharmacy inventory — namely non-moving items (dead stock) and stock held in excess of need — which the existing VEN system was unable to detect, and to propose appropriate management strategies for each drug group in order to reduce risk and loss in the pharmacy inventory of a teaching hospital. Although the hospital had adopted VEN analysis for inventory management since 2020, in 2025 it still faced a near-expiry drug-exchange burden amounting to 10,644,348.40 baht. Method: A descriptive cross-sectional study using retrospective data from the hospital pharmacy information system was conducted. All 2,196 drug items with a total annual drug expenditure (ADE) of 5,735,175,285.67 Thai baht from January to December 2025 were included. ABC analysis classified drugs by cumulative annual consumption value based on the Pareto principle. FSN analysis classified drugs by the number of months with actual dispensing activity over 12 months. An ABC-FSN matrix was constructed by linking both datasets using item codes. Results: FSN analysis identified 1,797 Fast-moving items (81.8%), 285 Slow-moving items (13.0%), and 114 Non-moving items (5.2%). ABC analysis revealed that 273 Class A items (12.4%) accounted for 70.0% of total ADE (4,011,990,820 baht). The ABC-FSN matrix showed that Group A-F (270 items, 3,990,223,083 baht) required the highest level of inventory control, while Group A-S (3 items, 21,767,738 baht) comprised high-cost biologics requiring adjusted ordering policies. All 114 dead-stock items (10,155,936 baht) were distributed in Group C-N, with none in Groups A-N or B-N. Conclusion: The ABC-FSN analysis identified 114 non-moving (dead-stock) items in Group C-N out of 2,196 total items (5.19%), amounting to 10,155,936 baht, and identified 3 high-cost biologics in Group A-S that should follow a just-in-time ordering approach — items that VEN analysis alone could not detect. Integrating the ABC-FSN tool with VEN analysis is therefore a promising approach to elevate drug inventory management in teaching hospitals toward the highest precision and efficiency.
