Abstract

Because of being a leading cause of worldwide death, coronary artery disease (CAD) is considered as a crucial health problem. Although advances in the treatment of CAD at present have contributed to a decline in hospitalization admission and mortality, successful CAD management also depends on patients’ commitment to modify their lifestyles. However, associated with many factors, adherence to long-term CAD management may be neglected. More than 60 percent of persons with CAD were vulnerable to hospital readmission because they did not adhere to therapeutic recommendations.  As for encouraging persons with CAD to adhere to therapeutic recommendations, collaboration among members in a multidisciplinary team in collecting data, doing a plan, performing as a plan, and evaluating the results has been launched.
