Abstract

Chronic diabetic foot ulcers are a major complication in diabetic patients. Reducing loading pressure on the foot is the most important factor for the healing of neuropathic ulcers.  This off-loading technique can be effectively accomplished by using the total contact cast (TCC) for a limited duration.  However, TCC has not been widely utilized due to lack of knowledge, experience and expertise.  In addition, many patients are reluctant to use TCC owing to fear of wound infection and cosmetic cencerns.  This article provides a comprehensive review and describes the author’s experience with the successful use of the TCC for treatment of chronic diabetic foot ulcers.
