Abstract

extraperitoneal laparoscopic herniorrhaphy ( ELH ) with CO2  insufflation  เป็นการผ่าตัดรักษาไส้เลื่อนบริเวณขาหนีบ  (groin hernia) โดยใช้กล้อง (laparoscopic method)วิธีหนึ่ง  ซึ่งการผ่าตัดวิธีนี้   สามารถทำได้ด้วยการวางยาสลบแบบ general anesthesia  หรือ  การฉีดยาเข้าช่องไขสันหลัง (epidural  anesthesia )  บ่อยครั้งเรามักต้องเปลี่ยนจาก epidural  anesthesia เป็นแบบ  general anesthesia  เพราะปัญหาการซึมรั่วของ CO2  จาก  extraperitoneal space  เข้าสู่ช่องท้อง  (pneumoperitoneum) เพื่อหลีกเลี่ยงภาวะดังกล่าว  ของรายงานผู้ป่วย 2  รายที่ผ่าตัดโดยวิธี  ELH  without  CO2  insufflation

ELH  with CO2  insufflation is an alternative to the laparoscopic method for treatment groin  hemia. The procedure is usually done with general  anesthesia or epidural anesthesia. Previously, if the peritoneum was perforated in a patient on whom epidural anesthesia was used, conversion  to general anesthesia was required because of the resulting pneumoperitoneum.                                                                                                                              The following is a description of the first  2 reported cases and technique of ELH without CO2 insufflation
