Editorial Note
To continue the thrive to excellence as a member of the ASEAN Citation Index (ACI) database, the Thai Pharmaceutical and Health Science Journal has been publishing all articles in English for the second issue of this year. We still have caught attention from research focusing on international health problems.
In this second issue of volume 11, the study entitled “Relationships between Protective Factors and Mental Health of Adolescents in Mongar, Bhutan” demonstrated a rigorous study conduct and an alarming mental health not only the adolescents but the family members, the community, and the society as well. The study poses a vital question, as every society’s future is in the hand of the new generation. The study findings could also trigger in-depth consideration in each of every society with shared culture, norm, and value, if not entirely comparable.
The study in Thailand on the Myanmar immigrants in the Thailand-Myanmar border province of Kanachanaburi reflects beyond pharmacy-related issues. In addition to the roles and responsibilities of community pharmacy to be identified and the problems that had been identified and solved, the problems of taking care of these Myanmar immigrants are still considerably non-manageable. In this era of global workforce migration, problems associating with these immigrants are undeniable. Fortunately, the study implied some hope that community pharmacy or drugstores could be a promising partnership in healthcare system. The governmental healthcare sector should not turn away from this fact. Various cooperative services could be put in place at the community pharmacy. These include screening service, referral service, and community visit for follow-up. If available, a seamless healthcare service could be achieved in a near future. A better health status of these immigrants could result in a better work force, less economic burden in the long-run and a less societal problem.
The other two studies involving caregivers of the patients, a growing concern in the coming years of elderly-dominated society. The more the number of the elderly, the more the illnesses that come with age. As a result, more family members to become the caregiver. The burden is now growing to the family caregivers. The illness of a family member causes stress, both physical and mental to the caregivers with various extent. Any intervention or program to help the caregivers cope and manage the challenge effectively could result in a preserved or even improved quality of life of theirs. The first study aimed to explore the health promoting behaviors of the caregivers of heart failure patient while the second one tested the effects of the program to enhance the caregivers’ skill in coping with stress and burden in taking care of patients with schizophrenia. In addition to previous research, these two studies not only add to the literature the findings on how critical interventions on the caregivers of patients with chronic illnesses are, but also suggest ways to develop practical interventions, if not merely a perfect one.
In this challenging endeavor of the Thai Pharmaceutical and Health Science Journal, we are hopeful to better the quality of the articles published. We urge more submissions from international research community, regional and global. We would like to thank in advance for any prospective submissions.

In this first issue of volume 14, the Journal has been proudly presenting five studies. The disciplines and issues of these research papers were somewhat diverse from laboratory work, to clinical and community care practice.
For the laboratory work, a quest for antioxidants goes above and beyond the usual sources, plants, to microorganisms. The study entitled “Antioxidant Capacities and Total Phenolic Contents of Selected Actinomycetes sp.” found a promising source of antioxidants in the fungus-like Actinomycetes bacteria. This source of antioxidant could be more efficient than the plant source since it could be harvested in laboratory which does not use up that much space. However, more studies on a broader range of microorganisms with potential antioxidant synthesis and more studies are needed for a thorough understanding for industrial production.
The second article is about herbs and nutritional supplements but dealt with the use of these products in patients with renal impairment. The study entitled “Effects of Patient Education on the Use of Herbal and Nutritional Supplements in Patients with Renal Impairment” examined a pharmacist-based education intervention to improve the knowledge and practice of the patients in using herbs and nutritional supplements. The found a significant short-term benefit of the educational program on knowledge and practice. Even though, the intervention did not improve the kidney function, that is no surprise since kidney function cannot be reversed anyway. Unfortunately the recent scientific knowledge could not help improve the declining kidney function, just mere maintaining it.
Another study involving the use of medication entitled “The study Factors Associated with Medication Adherence Behavior among Hypertensive Patients” explored more in-depth understanding on various psychological factors. Enabling and reinforcing factors associating with the adherence to continuous medication use were elaborated. The findings could be useful in creating interventions based on these factors to promote medication adherence among hypertensive patients which need a life-long treatment.
The last two studies involved psychological aspects of patient care. The study entitled “Symptom Distress, Need for Care, and Nursing Care Received in Hospitalized Palliative Care Patients” helped pave a better way to take care patients who need palliative care. At present, it could not be more relevant since patients with terminal illnesses from cancer and others are so prevalent. More studies on palliative care should be conduced with a broader range of aspects of care, and not only with nurses but caregivers and family members as well. More understanding on the care for this group of patients could help alleviate the stress among all parties involved.
The last study entitled “Factors Influencing Self-management of End-stage Renal Disease (ESRD) Patients Undergoing Hemodialysis in Bhutan” helped healthcare providers understand more about how to approach and care for these ESRD patients. The findings could be applicable not only to Bhutanese context but other countries in the region that share similar cultures, norms and values and as well. Since more ESRD patients have been seen in worldwide, the findings could be useful at a global scale as well. This group of ESRD population poses a huge burden, economically and psychologically, to all parties involved. More understanding could bring about a better care and hence a less burden.
The review article entitled “Acute Coronary Syndrome: Challenges of Emergency Nurse in Critical Care” is mainly for nurses. This is much needed article showing fine-tuned steps of patient care in the intensive and semi-intensive care setting.

In this second issue of volume 14, the Journal has been proudly presenting five studies. The disciplines and issues of these research papers were somewhat focusing on pharmacy and its related practice and policy.
In the rising time of international trading, the import and export of pharmaceutical products has been increasing. The integrity of the quality has never been more acquired before. The government agency responsible for this quality control has been trying to improve relevant process with some difficulties and not much success. The article entitled “Thai FDA Development of GMP Accreditation Approval System for Oversea Manufacturers” sheds the light on the whole process along with the obstacles and some proposed solutions. This analytics on the process of bureaucratic work is fascinating yet worrisome for the development of the country. However, more studies on the ongoing development process of GMP accreditation approval on oversea manufacturers are highly needed.
Kidney failure is an ever-rising health problem worldwide. A certain number of interventions, preventative and curative, have been attempted and have been proved for certain level of success. The study entitled “Factors Influencing Patient Health Behaviors for Delaying the Progress in Stage 3 - 4 Chronic Kidney Disease Patients at Khlongkhlung Hospital, Khampangphet Province” is a part of a grand-scale project to counter kidney failure in Thailand. This kind of study is also critical for the foreseeable success and obstacles of the project. It enriches the understanding of human as socially living organism which could further help modify the future interventions to be more successful. The behavioral science behind it is of standard thought compatible with the academic norms. However, more studies on other behavioral concepts emerging from the ground are challenging and much needed.
One of the most long-lasting and ever-growing in Thailand public health is the use of the pre-packed polypharmacy or Ya Chud available conveniently throughout the country. The problem has been increasing in the extent and severity because of a loose law enforcement and a lack understanding and more importantly awareness/realization of the associated dangers. The study entitled “The Relationship between Knowledge and Use Behavior of Polypharmacy or Ya Chud in a Community in Srisaket Province” again demonstrates the extent of the problem in a local community. The knowledge on the matter is still low and the use behavior (or reasons to use) are in moderate level. Since the higher level of the use behavior in the study means the more impression of improper reason to use, this moderate level of use behavior is somewhat worrisome and needs an urgent clarification and understanding through educational programs or campaigns. It is fascinating that the problem of Ya Chud is even widespread throughout the country. At this point, even predictive studies are not much useful in contributing to the problem solving. We need more intervention studies to prove the effectiveness of the problem-solving actions.
Last, the two studies examining efficacy of bisphosphonate and non-bisphosphonate drugs in preventing bone fractures in postmenopausal women with osteoporosis by means of meta-analysis. The studies could be beneficial for a rational drug selection. We have been seeing this kind of studies with the rising demand of evidence-based policy making on national essential drug list policy.

In this third issue of volume 14, the Journal has been proudly presenting five studies. The disciplines and issues of these research papers were somewhat diverse from traditional medicines, to clinical practice, and teaching.
As a substantial health burden, hepatitis B needs a more effective therapy. The study entitled “Lamivudine as Initial Treatment for Chronic Hepatitis B Virus in a University Teaching Hospital: a Retrospective Patient Record Review Study” revealed effectiveness of lamivudine in a real-life practice. The investigators found that the use of lamivudine 150 mg as initial treatment for chronic hepatics B virus could achieve undetectable levels and ALT normalization. However, a small number of patients had HBe seroconversion and 58.2% needed additional antivirals. This moderate achievement in hepatitis B treatment brought some hope and led for the future research for this ailment.
The second study on developing a pharmacist-involved multidisciplinary team to take care of patients with non-communicable disease (NCD) at a community hospital level is much needed. Most previous studies emphasized on the quantitative outcomes of drug-related problems relieved. On the other hand, the present study provided more detail on how to develop the team. All obstacles found in the developing process of this study could be beneficial for other healthcare settings in implementing the NCD clinic with pharmacist participation.
The study entitled “The Use of Medicinal Plants for Gynecologic Ailments by Thai Traditional Folk Healers in Nakhonnayok Province” provides the insight of traditional medicines based on herbs. The link between scientific enquiries and the depth of Thai traditional herbal medicine has been evident. In addition to the relevant matters between scientific findings and traditional medicine, the realization of the need to fully understand the Thai traditional medicine before one could make such as link is crucial. Otherwise, such link could turn out to be merely a hoax.
The fourth study entitled “Effects of Thai Traditional Play Program on the Ability to Control Muscle and Reaction Time of Muscle among Female Students in the Primary School” demonstrates an effort to promote health by reliving the traditional Thai plays. Since all play modalities tested emphasized the movement of the legs, the ability to control muscle and reaction time were improved immensely. This study was a proof the traditional wisdom in any given culture has benefit its people regardless of time and worthy reserving and promoting for practical use.
The last study is another effort to improve learning experience in pharmacy. The study entitled “Effectiveness of Computer-Assisted Instruction of the Pharmacology of Anticancer Drugs” showed a comparable effectiveness of a CAI of pharmacology of anticancer drugs to that of the traditional handout in the two self-study sessions after the usual lecture class. Even though not better than the traditional handout for self-study sessions, CAI could offer a more relaxing learning experience with immediate interactive feedbacks. In the era of communication technology, more learning modalities could be created and tested for better learning effectiveness.
Lastly, the review article entitled “The Colony-Stimulating Factors and Febrile Neutropenia” has offered the hope that more advanced biosimilar products could be more available for Thai patients.

In this fourth issue of volume 14, the Journal has been proudly presenting five studies. The disciplines and issues of these research papers were somewhat diverse from pharmaceutical laboratory work, to clinical practice.
With the effort to improve a release profile of commonly used drugs, the study entitled “Effect of Three Natural Polymers on the Release Kinetics of Propranolol HCl from Hydrophilic Swellable Matrices I: Matrices Containing Microcrystalline Cellulose” serves the purpose well. A better release profile could provide a more sustained serum level of propranolol to improve therapeutic effect with less adverse effects. The knowledge could be a basis for further development of pharmaceutical products of Thailand and others as well.
Warfarin has been one of the drugs with the most concern on therapeutic and safety effects with its narrow therapeutic index nature. Difficulties in adjusting dose of warfarin to achieve target therapeutic range for respective indications have been a challenge in the real clinical setting. A clinical study comparing effects of pharmacist-managed warfarin therapy with usual care entitled “Effects of Pharmacist-Managed Warfarin Therapy at Mahosot Hospital, Lao PDR” has added the findings to the literature. The findings help confirm the pharmacist contribution in managing warfarin use.
Obesity has become a more prominent problem in Thailand and worldwide, especially in the children. Various strategies have been used to tackle the problem with some extent of success. The study entitled “Effects of Family-based Intervention to Prevent Overweight in School-age Children” demonstrated an effort to improve health status of school-age children. More studies with a varieties of activities and concepts should be explored so that a set of the most effective programs or activities could be identified. Children’s health is worth investing.
Foot ulcer is one of major microvascular complications of diabetes. Various strategies have been tested and implemented in preventing foot ulceration, and foot amputation. The study entitled “Effects of Foot Health Promotion Program on Foot Care Behavior of Type 2 Diabetic Patients in Muang District, Sukhothai Province” demonstrated the effectiveness and feasibility of the program in the real clinical and community setting. Elements of the program suitable for the community were tested. More studies suitable for different communities should be conducted.
The last study is an effort to improve obstetric care in pregnant women receiving care at ante-natal care of a tertiary hospital. The study entitled “Effects of Portable Pocket Diary for Recording Fetal Movement Counts Compared with the Usual Counting Diary on Consistency of the Counting” could provide a better ANC care for the fetus and newborn.

In this first issue of volume 15, the Journal has been proudly presenting five studies. The disciplines and issues of these research papers were somewhat diverse from applied laboratory work, to clinical practice both for pharmacy and nursing.
In the emerging era of antimicrobial resistance, the study entitled “Development of the Supporting and Promoting System for Appropriate Antibiotics Use in a Community Hospital by Concept Mapping Techniques with a Multidisciplinary Team” could not address the crisis better. The critical role of multidisciplinary team to tackle the problems was elaborated especially the information technology aspect the of task. The study could have encouraged more hospitals to start and strengthen their multidisciplinary team in rational antibiotic use.
In line with the study previously mentioned, the study entitled “Patients’ Perception towards Treatment Efficacy of Upper Respiratory Infection and Acute Diarrhea in Primary Care Units” emphasized the importance of the patient’s perception on the treatment for common illnesses with no use of antibiotics. They found that the positive perception existed at a satisfactory level. The compliance was also associated with the perceived efficacy. The study shows a promising future of rational antibiotics use in primary care settings.
The study entitled “Voriconazole Dosing Recommendation Based on CYP2C19 Phenotype” shows an attempt to optimize dosing of the drug voriconazole for invasive fungal infection. This fine-tuning is of highly critical need. More studies on the issue should be encouraged for better treatment for critically ill patients.
The study entitled “Factors Affecting Kidney Failure in Patients with Type 2 Diabetes Mellitus in Banna Community Hospital, Nakhonnayok Province” expanded knowledge on the performance of community hospital in handling diabetes patients with various kidney failure risk.
In nursing, the study entitled “Effects of Music-based Group Reminiscence Program on Depression in the Elderly at Social Welfare Development Center for Older Persons” could have not been suitable for the elderly-emerging era. More options to help improve psychological health among the elderly is highly appreciated.

In this third issue of volume 15, the Journal has been proudly presenting research articles of various disciplines. For pharmacy practice, the study entitled “Prescription Patterns and Clinical Outcomes of Antiplatelet Therapy in Acute Coronary Syndrome with End-Stage Renal Disease” shed the light on how well the use the antiplatelet therapy in a special group of patients, those with acute coronary syndrome and end-stage renal disease. The issue of suboptimal use has been known worldwide, and such phenomena in Thailand is also of great concern.
In the second study on pharmacy practice entitled “Incidence and Clinical Features of Anti-epileptic Drug Related Adverse Drug Reactions in Chiangrai Prachanukroh Hospital, Thailand,” the effort to prevent adverse events from anti-epileptic drugs has been evident. The authors and their institute have pioneered and advanced all aspects of pharmacy practice and we hope to welcome more of their publications.
A study in nursing practice entitled “Effectiveness of a Motivational Enhancement Group Therapy Program on Drug Abstinence Intention Among Thai Amphetamine-Abusing Youths” could have encouraged more advancement in helping young substance abusing addicts.
On the technology side, the two studies entitled “Propranolol HCl and salicylic acid-loaded hydrophilic and hydrophobic colloidal silicon dioxide anhydrous gels” and ”HPLC Analysis and Solvent Extraction of Emodin from Ventilago denticulata Willd” are the testament for scientists on improving products and refining scientific procedures.

In the era of elderly population and patients, the study entitled “Factors Related to Health Behaviors of the Elderly with Recurrent Stroke” helped understand more and could have led to a better self-care for the elderly with stroke. The study entitled “Effects of Collaborative Drug Therapy Management Protocol Use for Controlling Blood Sugar levels of Patients with Diabetic Kidney Disease Visiting Diabetic Clinic of a General Hospital” emphasizes the collaborative work between professionals for a more effective healthcare.
The study entitled “The Risk Assessment and Effects of Education for Cardiovascular Disease in Women Using Combined Oral Contraceptive Pills” could have offered the concern on the role of community pharmacist in taking care of women customers.

In this second issue of volume 16, the Journal has been proudly presenting five studies. The disciplines and issues of these research papers were somewhat diverse from applied laboratory work, to clinical practice both for pharmacy and nursing.
Interventions to better modify health behavior and lifestyle among type 2 diabetes patients have always been investigated. The research entitled “Effects of Pharmaceutical Care with food Education According to Isan Life Style in Patients with Type 2 Diabetes Mellitus” showed an intervention catered to local lifestyle. This proves again that one-size-fits-all interventions would never offer the best outcomes for diseases like diabetes that need lifestyle related management. In the study, the intervention was delivered by pharmacists. However, we believe that the intervention could be rendered by other professionals as well which could be considered advantageous both for the provider and the patient.
Another intervention to improve diabetic outcomes is the study entitled “Effects of the Resilience Promoting Program on Self-management Behavior and Hemoglobin A1C Level among Older People with Diabetes.” This psychology-based intervention delivered by nurses also deserved an appraisal.
With an immense burden on all levels of society, studies about cancer are always of great contribution. The study entitled “Selected Factors Related to Comfort of Advanced Cancer Patients” helps broaden viewers’ view on what providers should be concerned in providing comfort for advanced cancer patients.

Editorial Note
In this third issue of volume 16, the Journal has been proudly presenting 12 studies. The disciplines and issues of these research papers were somewhat diverse from applied laboratory work, to clinical practice both for pharmacy and nursing, and policy related issues.
For psychosocial interventions, the study entitled “Feasibility of the Motivational Interview based Adherence Therapy (MIAT) with Family Support Program to Promote Medication Adherence for Persons with Schizophrenia” emphasized the motivational interview on improving medication adherence among schizophrenia patients. Medication adherence in this group of patients has been known to be a major factor for the success of controlling the illness. Contribution of this study is considerable.
For more clinical outcomes, the study entitled “Effects of Training Program by Multidisciplinary Team on Knowledge and Practice of Patients or Caregivers for Infection Prevention and Drug Therapy Problems in Peritoneal Dialysis Patients in Jainad Narendra Hospital” emphasized crucial role of multidisciplinary team to tackle infections among home-based CAPD patients. Since this group of patients has been expanding, the study could contribution enormously to contain healthcare burden.
The study “Factors Affecting Renal Adverse Reactions in HIV Infected Patients Receiving Tenofovir Disoproxil Fumarate at Damnoen Saduak Hospital” explored the risk of this effective antiretroviral drug on its kidney toxicity in Thai patients. With its retrospective nature of the study, it could still signal the risk and prompt more studies regarding the growing crucial issue of renal impairment which could be substantial since the drug has been increasingly used.
For studies on practice policy related issues, the study entitled “Perceptions on Safety Culture among Hang Dong Hospital Personnel, Chiang Mai Province: A Qualitative Study” is in a great need since creating safety culture is highly crucial for the success in adverse events prevention. The study named “Enriching Work Value among Hospital Pharmacists in Nan Province: A Qualitative Study” reflects the need to enhance the importance developing pharmacy profession within the context of the policy of budget constraint which could limit hiring pharmacist.
In an effort to use shellac wax base, the study entitled “Thermal Properties and Crystallinity Behavior of Hydrochlorothiazide and Propranolol HCl Incorporated in Shellac Wax-Poloxamer Matrix Bases” proved the promising use of this basic pharmaceutical material that could be found abundant in Thailand. Another study to enhance market value of local active ingredient is the “Dissolution Improvement of Curcumin by Solid Self-emulsifying Drug Delivery System (Solid-SEDDS).” The findings could be useful for product development.

In this second issue of volume 17, the Journal has been proudly presenting 12 studies. The disciplines and issues of these research papers were somewhat diverse from education, applied clinical practice to policy both for pharmacy and nursing.
The study entitled “Learning Motivation of Thai Pharmacy Students” reflects the present learning difficulties among undergraduate pharmacy students. Challenges are presented for improvement. The challenges in practicing pharmacists in public hospitals are also presented in the study “Needs and Attitudes of Thai Hospital Pharmacists on Continuing Specialty Education and Training.” This struggle of hospital pharmacy practice in the healthcare system is evident. This trend of specialty education is influenced by the United States hospital pharmacy practice. Whether it is worth of pursuing will be determined in the near future.
The use of mobile phone applications has been more popular among the elderly patients. The study “Descriptive Model of a Self-care Mobile Application for Elderly Patients with Hypertension” demonstrates how to develop a practical application with inputs from all stakeholders. In the Covid-19 pandemic, the study “Factors Affecting COVID-19 Prevention Behavior in Community Pharmacies in Chonburi Province, Thailand” reflects the crucial roles and responsibilities of community pharmacy in providing care in everchanging circumstances. Experiences from this pandemic could be useful in handling future catastrophe.
