The characteristics of psychological factors and personality in young male smokers

The purpose of this study was to examine the health status and personality of young male smokers who have smoked everyday for one year. A total of 120 participants were examined with the personality questionnaire MMPI and the health questionnaire GHQ28. Data analysis was done using the average percentage and standard deviation of the T-score. Our studies showed that the subjects had a personality trait that avoid facing reality had a tendency to isolate and be fantasy (SC value T = 73). They were depressed, unsatisfied with their lives, had a pessimistic world view, and were self-centered (D and HS value T = 70). Furthermore, examination of the mental health status using the Thai GHQ 28 found that 37.6% were depressed, and 49% had anxiety and sleep difficulties. In addition, 10.8% had somatic problems and 2.5% had social withdrawal. Considering the period of start smoking, 55.83% started smoking at the age of 13 to 15, 35% at the age of 16 to 18, 5.83% at the age of 19 to 21 and another 3.33% at the age of 22 to 24. Among the factors initiated smoking behavior, introduction by friends accounted for 68.3% of the participants, self-experimentation for 12.5%, and having smoked family members for 19.2%. As for the intention to quit smoking, 80.8% of the participants who tried to quit smoking failed while 19.2% never tried.

Prevalence of obstructive sleep apnea in pregnancy

The aim of this study is to determine the prevalence of obstructive sleep apnea in Thai pregnant women. The study was carried out from January 2010 through December 2010 at antenatal clinic, HRH princess Maha Chakri Sirindhorn Medical Center, Nakorn-nayok province. One hundred and twelve pregnant women were enrolled. All subjects answered a Berlin questionnaire, which had information on age, gravida, gestational age, prepregnancy body mass index, snoring and sleep apnea. The subjects who answered snoring or sleep apnea were tested by polysomnogram for detecting obstructive sleep apnea. The result showed no obstructive sleep apnea was detected. However, the prevalence of sleep apnea from the questionnaire was 3.57% and the prevalence of snoring in pregnancy was 31.25%.

Etiology of upper gastrointestinal bleeding at HRH Princess Maha Chakri Sirindhorn Medical Center

Upper gastrointestinal bleeding (UGIB) is a common emergency condition in general practice.
No data are available about the etiology of UGIB at HRH Princess Maha Chakri Sirindhorn Medical Center. The purpose of this study was to evaluate the etiology of upper gastrointestinal bleeding at HRH Princess Maha Chakri Sirindhorn Medical center. We conducted a descriptive study of the patients who presented with UGIB and underwent esophagogastroduodenoscopy (EGD) at HRH Princess Maha Chakri Sirindhorn Medical center from June 2006 to December 2008. A total of 111 patients were enrolled.
The patients age ranged from 15 to 88 years and male to female ratio was 2.5:1. The causes of bleeding could be detected in 98.2% by EGD. The lesions were found in the stomach, duodenum and esophagus for 71.2%, 15.3% and 11.7% of the patients, respectively. Peptic ulcer (gastric ulcer & duodenal ulcer), gastritis and variceal bleeding were the most common causes of upper gastrointestinal bleeding in 66.7%, 14.4% and 8.1% of the patients, respectively. EGD is a safe and useful procedure and should be used as the primary diagnostic approach in patients presented with upper gastrointestinal bleeding.

A study of the satisfaction from users and service providers in the pathology laboratory

The pathology laboratory of HPH Princess Maha Chakri Sirindhorn Medical Center will come to laboratory quality system certification standard work. Laboratory satisfaction was studied in users and service providers. This Study determined the satisfaction of users and service providers in the pathology laboratory in 2009 – 2012. Participants conducted a self-directed study of the pathology laboratory. Data from outpatient users (n=110) include environment, location, speed, consideration, process of service, benefit and overall satisfaction. Data from staff-medical users (n=120) include accuracy, speed, price, diversity of service, location, and overall satisfaction. Data from service providers (n=46) include safety, salutary, progression, salary and relationship of service provider. Overall satisfactions to the pathology laboratory and blood bank from outpatient users are 71.08% and 90.38% respectively. Overall satisfaction with the central laboratory, blood bank, anatomical pathology and emergency laboratory from staff-medical users are 70.17%, 72.53%, 72.35% and 70.90% respectively. Overall satisfaction with the pathology laboratory from service providers in safety and salutary is 74.43%. Overall satisfaction with the pathology laboratory from service providers in progression and salary is 69.88% and 71.79% in the aspect of the relationship of service providers. The hospital administration should improve the considerable and the responsibility of service provider, speed and diversity of the laboratory and salary and stability in career path for service providers.

Physical health problems in children using computer

Computers are being used by children in Thailand and many countries around the world. There are increasing reports of a relationship between increased musculoskeletal disorders in children and the computer users. Children reported musculoskeletal pain can be occurred at neck, back, shoulder, arm, wrist and hand. During using computer, sustained posture and repetitive contraction are main reasons for the risk of muscular pain. It was found that some children reported muscular pain at mild level, however in some cases, muscular pain was found at moderate and severe level during using computer. The causes of the musculoskeletal problems are type of computer, poor workstation, the large size of children head compared with body, and the way to interact with computer during using computer which is different from adult. Guideline for children using computer is that children should sit with the feet fully supported on the floor with knee bending at 90 degree. Keyboard and/or mouse should be placed at the elbow height and upper arm should be relaxed beside the body and the keyboard is under the fingers. Finally, the cause of physical health problems especially in musculoskeletal problems during using computer has been found in children. Clinician, parents, teachers and children should consider and work together to prevent this adverse health problems.

The mental health promotion evaluation form for depressive disorders prevention in elderly in Chiang Rai who attended the 3 months trial plan of the form of mental health promotion under local culture. This consisted of Tung tree cutting, prolonging activities and stimulation, Bai Sri Su Khwan, recycling and Tai Chi exercise. This study used a simple random sampling with a list of 36 subject names. The research instruments were the evaluation form under the Ottawa charter for health promotion which consisted of creating a policy for mental health promotion, creating an environment that is conducted to mental health well-being, strengthening the community and stakeholders in mental health promotions, developing personal skills in promoting mental health and modifying a health care system for promoting mental health. To evaluate the satisfaction using this form were determined by 5 experts for the content validity and the results were 0.93. This study found that health promotion form was at a high level with the mean 4.41 (SD = 0.49) and the satisfaction of the elderly who participated in the trial form was at a moderate level with the mean at 3.67 (SD = 0.30). Therefore, the creation process of the form of mental health promotion for depressive disorders prevention in the elderly with severe level based on their social culture, and understanding their distress, happiness and requirements with emphasis on activities promoting health and reducing the risk of depression in these elderly.

Tools for fully automated volumetric measurement of cerebral atrophy from MRI ready for routine clinical practice in diagnosing Alzheimer’s disease

Hippocampal atrophy is the best validated biomarker of Alzheimer's disease (AD) and can be detected quantitatively by volumetric magnetic resonance imaging (vMRI). Fully automated vMRI software was developed to overcome many clinical translating hurdles of the manual and semi-automated methods. However, there are no data of sensitivity and specificity of vMRI to be used as a tool to diagnose AD in general clinical setting. Moreover, the hippocampal atrophy can be found in various neurological conditions with memory impairments, not only the AD. This hippocampal atrophy results from vMRI should be accompanied by/with the clinical evaluation rather than be used alone.

The practice guideline for preventing and caring of foot ulcer and lower limb amputation in diabetic patients can be divided into 2 categories: 1) the practice guideline to prevent foot ulcer for patients who have no foot ulcer. This guideline comprises foot examination, assessment of foot ulcer, differentiation of risk categories, and preventing foot ulcer in each risk category. 2) the practice guideline to treat foot ulcer and prevent lower limb amputation for patients who have foot ulcer. The guideline for this category includes classification of foot ulcer severity, and assessment of foot ulcer risky to lower limb amputation, and prevention of lower limb amputation in each level of foot ulcer severity. These practice guidelines are useful especially for nurse practitioners who take responsibility in primary care service for effectively caring of foot ulcer and preventing lower limb amputation in diabetic patients.

Prevalence of driving and alcohol consumption of students in the Ongkarak secondary school

Traffic accidents are the major cause of injuries and deaths in Thailand. Investigating risk factors can assist planning and prevention of these accidents. Questionnaires were answered by secondary school students in Ongkharak School. From 88 students, there were 50 girls (56.82%) and 38 boys (43.18%). The age ranged from 12 to 15 years with a mean age of 14.17 years. A history of driving motor vehicles was observed in 73.9% of all students, of which 78.5% were motorcyclists and 20% were car drivers. Alcohol consumption was found in 34.1% of all students. The mean age of drinkers was 14.53 years. and the majority of students (81.5%) were drinking only in special occasions. Of all alcohol consumers, 66.7%, 16.7% and 13.3% drank with friends, seniors and relatives, respectively. There were only 10% of students who were driving or motorcycling after drinking alcohol.
